NYSWDA is collecting the state BLL's as one more piece of the OSHA NID for your protection. This data will hopefully establish that our LSW practices work and that weatherization work is indeed safe for our employees. You can fill out this form or wait for a hard copy of the form to arrive via mail.

Year, SS# of employee, Lead in Blood #, zinc #, # years in WAP (important to know how long they have worked in weatherization)
